摘要 目的:探讨脂肪源间充质干细胞(adultadiposetissue?derivedmesenchymalstemcells,AMSC)治疗急性移植物抗宿主病(acutegraft-versus-hostdisease,aGVHD)的机制。方法:3例行异基因造血干细胞移植术后发生aGVHD患者,以每公斤体重2×106个细胞剂量静脉输注AMSC;首先应用尼龙毛柱分离外周血T淋巴细胞,再经CD4或CD8免疫磁珠分选出CD8+或CD4+T淋巴细胞,应用流式细胞术检测发生aGVHD患者使用AMSC前后外周血CD8+及CD4+T细胞亚群的变化。结果:与输注AMSC前相比,输注AMSC后,CD8+T细胞中的CD8+CD28CD25--亚群显著上调,CD4+T细胞中的CD4+CD25+亚群并无明显变化,同时,患者的aGVHD得以有效控制。结论:AMSC治疗aGVHD的作用机制可能与其上调CD8+CD28-T细胞亚群有关,而与CD4+CD25+亚群可能并无直接关联。 Objective Toinvestigate the molecular mechanism of treatment with mesenchymal stem cells (AMSC)on acute graft-versus-host disease (aGVHD) patients. Methods Adose of 2 × 10^6 AMSC perkg of the aGVHD patients weight was given intravenously. T cells were harvested by using nylon column, then CD8^+ or CD4^+ T cells were sorted by magnetic beads, respectively ,then the proportion of CD8^+CD28^- Tcells or CD4^+CD25^+ T cells was assayed by fluorescence-activated cell sorter (FACS) in 3 cases with aGVHD following allogeneic hematopoietic stem cell transplantation,respectively, Results The data of FACS indicated that in the CD8^+ T cells, CD8^+ CD28^-T cells were up-regulated significantly after AMSC transplant,while the fraction of CD4^+CD25^+ T cells within the CD4^+T-cell compartment had no significant change, At the same time, a complete remission of aGVHD disease was observed. Conclusion The mechanism for the successful treatment with AMSC on aGVHD might be correlated with the up-regulation of CD8^ CD28^-T cells in rive;but the frequency of donor CD4^+CD25^+ T cells showed no significant correlations with the clinical outcome of MSC.
