摘要
目的调查健康中、老年人载脂蛋白(apo)AI、AⅡ、B、CI、CⅡ、CⅢ、E及脂蛋白(a)[Lp(a)]的参考值,分析各项血脂与apo之间的相关。方法严格选择生活水平一般的北京健康居民,男225人,女58人,年龄38~83岁(大都在45~70岁间)。血脂测定用标准化方法,apoAI、B参考血清的定值符合WHO国际标准。除Lp(a)与apoCI测定用ELISA法外,其他各项apo分析均用免疫透射比浊法。结果统计得各项apo的均数与百分位数分布并与国内外资料比较。相关分析显示Lp(a)与apoCI是独立的变量,与年龄、体重及各项血脂均无明显相关。其他各项血脂与apo指标可以分为3组,分别代表高密度、低密度与极低密度脂蛋白水平。分析了体重指数、甘油三酯与高密度脂蛋白的相互关系。结论apoAI、B测定中采用了WHO国际参考血清后结果比以往的报告有明显提高。其他apo测定尚未开始标准化研究,但作者所列参考值大致与用同类方法所报告的数据近似。14项血脂之间的相关分析。
sObjectivesToinvestigatetheeferencevaluesofapoAI,AⅡ,CIⅠⅡ,CⅢ,Eandlipoprotein(a)ofmiddleagedandoldsubjects,andtostudythecorrelationsbetweenvari-ouslipidparameters.MethodsAfterthoroughphysicalandlaboratoryexamination,225maleand58femalehealthyBeijingresidents(aged38-83,mostly45-75yearsold)withmoderatelivingstandardwereselected.Serumtotalcholesterol(TC),lipoproteincholesterol(HDL-CandLDL-C)andtriglyceride(TG)wereanalyzedwithwelstandardizedmethods.AssignedvaluesofapoAI,BofserumcalibratorswereprovedtobetrceabletothetargetvaluesofWHOinternationalreferencematerials.Mostaposweredeterminedbyimmunoturbidimetricmethods,butLp(a)andapoCIweredeterminedbyELISAmethod.ResultsAverageapoBlevelswere1.01g/Linmenand1/07g/Linwomen,thecorespondingTCandLDL-Cvalueswere4.87and2.95mmol/Linmen,and5.16and3.13mmol/Linwomenrespectively.MeanapoAIlev-elswere1.42g/Linmenand1.45g/Linwomen,andthecorrespondingHDL-Cwas1.41and1.54mmol/L.TheproportionofHDL2-CandHDL3-Cwasapproximately4:6.ThemeanandmediumvaluesofLp(a)inmaleswere144and107mg/L,infamaleswere154and117mg/L.Themeanandthepercentiledistributionoftheaposstudiedwerelistedindetail.CorrelationanalysisrevealedthatLP(a)andapoCIwereindependentvariables.Otherparame-tersweredividedinto3groups;theymainlyrepresentedthelevelsofHDL,LDL,andVLDL.BodymassindexwasnegativelycorrelatedwithHDL-C,andpositivelycorrelatedwithTG.TGshowedclosenegativecorrelationwithHDL-C,apoAIandapoAⅡ.ConclusionsAl-thoughstandardizationstudyonotheraposhasnotstartedyet,theobservedreferencevaluesofLP(a),aopAⅡ,CsandEweresimilarofthosereportedwiththesamekindofanalyticalap-proach.Simultaneusdeterminationof14lipidparmetersandanalysisoftheircorrelationsmaycontributetothestudyofrelatedmetabolicproblemsandproperselectionoflaboratorytestsforclinicaluse.
