儿童卡波西水痘样疹46例临床分析

Retrospective analysis of 46 cases of Kaposi's varicelliform eruption in children

目的:探讨儿童卡波西(Kaposi)水痘样疹(KVE)的临床特征和易感因素,以及KVE的合理治疗。方法:对46例KVE患儿的临床资料进行回顾性分析,并与无KVE的特应性皮炎(AD)患儿进行比较。结果:KVE患儿在AD首次发病年龄、血清总IgE水平方面与对照组相比差异无显著性,而在发热、CD4+细胞百分率、CD4+/CD8+值下降方面与对照组相比差异有显著性。另外,阿昔洛韦联合静脉注射用人免疫球蛋白(IVIg)治疗在退热、止疱天数上要明显短于单用阿昔洛韦治疗。结论:KVE患儿具有高热、细胞免疫低下的特点,AD首次发病年龄早、血清总IgE水平高并非是易感因素,阿昔洛韦联合IVIg治疗有利于及时控制KVE急性期的病情。

Objective：To investigate the clinical features and predisposing factors of Kaposi＇s variceliform eruption（KVE） in children and to explore the appropriate approach in management of KVE.Method：A retrospective analysis of clinical data on 46 cases of KVE in children was conducted,and selected cases of atopic dermatitis（AD）as control.Results：The age at onset of KVE and the total serum IgE level in KVE were not significantly different from those in control group.Whereas fever,the percentage of CD4^＋ cells,and the ratio of CD4^＋/CD8^＋ cells in KVE were significantly different from those in control group.The therapeutic effect of acyclovir combined with intravenous immounolobulin （IVIg）was significantly better in treating fever and vesicopustules than acyclovir alone.Conclusions：Fever and low cellular immunity are associated with KVE in children whereas onset of AD and high total serum IgE level are not related to KVE.Acycloivir combined with IVIg seems to be valuable in the management of KVE.
