微创动力髋螺钉治疗高龄股骨粗隆间骨折

Therapeutic effect of small incision DHS fixation on intertrochanteric fractures of femur in elderly patients

目的观察微创动力髋螺钉治疗高龄患者股骨粗隆间骨折的疗效。方法2000年6月至2005年1月，采用微创动力髋螺钉内固定治疗80岁以上高龄患者股骨粗隆间骨折66例，术前充分准备并对其疗效进行随访。结果59例患者获随访1～4年（平均2．5年），7例术后1．5～3年死亡。关节功能优52例（80％），良5例（8％），差2例（2％），优良率为86％。结论微创动力髋螺钉内固定治疗高龄患者股骨粗隆间骨折，手术创伤小、卧床时间短、骨折愈合快、肢体功能恢复好、髋内翻等并发症少，是治疗高龄患者股骨粗隆间骨折的较好方法。

Objective To observe the therapeutic effect of small incision DHS on the intertrochanteric fracture of femur in eldarly patients. Methods From June 2000 to January 2005, 66 elderly patients with intertrochanteric fractures of femur （43 males, 23 females, age range： 80～93 years, mean 86 years） were treated with DHS. Careful preoperative preparations of 1 to 14 days were made. All the patients were followed up. Results Among 59 patients, the follow-up time lasted from 1 to 4 years （mean 2.5 years）, 7 cases died. The joint function 52 cases （80%） were excellent, 5 cases （8%） were good, 2 cases （2%）, were poor. The excellent recovery rate was 86%. Conclusion DHS was an ideal fixation method to treat intertrochanteric fractures of femur in elderly patients, with mild operative trauma, short time in bed, quick healing and functional recovery, few complications.
