急性大疱性鼓膜炎的临床报告

Clinical report of acute bullous myringitis

目的观察急性大疱性鼓膜炎的临床经过,探讨其有效疗法和并发症的防治。方法观察本院收治的35例大疱性鼓膜炎病例的临床诊疗过程,分析其听力恢复情况。结果痊愈32例(91.42%),好转3例(8.58%)。19例听力减退患者,其中6例为感音神经性聋,8例为混合性聋,5例为传导性聋;治疗结束时,16例患者平均听阈恢复正常(84.21%),3例于治疗1个月后平均听阈好转,但未达到正常水平(15.79%)。这3例中,2例是感音神经性聋,1例为混合性聋。结论经积极治疗后大疱性鼓膜炎病变可以痊愈,但极少数患者的听力可能难以恢复正常。

Objective To investigate effective therapies for the treatment of acute bullous myringitis and to explore the strategy for preventing the complications from occurring based on a clinical observation on the natural process of this lesion. Methods Thirty-five patients diagnosed as acute bullous myringitis in our Hospital were included in this study and their clinical data were carefully analyzed, with special attention focused on their recovering process of hearing loss. Results Among these cases, 32 （91.42%） were cured within 2 weeks and 3 （8.58%） were effective taken a longer therapeutic course. There were 19 with hearing loss in this group of cases before the therapy, 6 being neurosensory in nature, 8 being mixed and 5 being conductive. By the end of therapeutic course, 16 cases （84.21% ） had their averaged hearing thresholds recovered to the normal level and 3 （ 15.79 % ） with their averaged hearing thresholds improved following the therapy lasted for more than a month, two being neurosensory hearing loss and the other being mixed hearing loss in nature. Conclusions As a kind of self-limiting disease, acute bullous myringitis can be cured following the therapy, with great majority of patients fully recovered. However, it may be very difficulty to have the hearing recovered to a normal level in a few cases in spite of positive treatment.
