基质金属蛋白酶在急性心肌梗塞中的表达研究

摘要 目的 探讨在急性心肌梗塞患者外周血中基质金属蛋白酶-2(MMP-2)和基质金属蛋白酶-9(MMP-9)的变化以及MMP-2和MMP-9与左室重构的关系;方法:确诊急性心肌梗塞患者(AMI组,n=15),AMI组在确诊第1天(D1)、7天(D7)、14天(D14)、21天(D21)28天(D28)以及正常健康者(对照组,n=15)取外用血2ml,常规分离血清,利用Gelatin-PAGE法(Zymography法)测外周血MMP-2和MMP-9的活性表达;第28天超声心动图检查对照组和AMI组左室舒张容积大小,并分别与MMP-2、MMP-9做皮尔逊相关分析;结果:AMI组MMP-2和MMP-9比对照组高,P<0.01;随着时间的延长,MMP-2和MMP-9表达不一:MMP-9随着时间的延长而持续增高,MMP-2活性则在21天时达到最高峰,随后减少;MMP-2和 MMP-9的变化比心室重构发生的早,AMI组左室舒张末容量(LVEDV)比正常组大(P<0.01),而且左室容积与MMP-9有相关性。结论:MMP-2及 MMP-9可能参与心肌梗塞损伤,MMP-9可能参与心肌梗塞后左室重构的过程。 AIM To investigate the expression of Matrix metalloproteinases-2 (MMP-2) and Matrix metalloproteinases-9 (MMP-9) in acute myocardial infarction (AMI) and the relationship among MMP-2,MMP-9 and the remodeling of the left ventricular. Methods: 30 cases were analysis including 15 cases healthy people (control group) and 15 cases AMI patients( AMI group) .The changes of MMP-2 and MMP-9 expression of peripheral in each group were measured by Zymography The expression of MMP-2 and MMP-9 in AMI group were detected in 1^st day (D1) ,7^th day (D7) , 14^th day (D14) , 21^st day (D1) and 28^st day (D28) The left ventricular end-diastolic volumes (LVEDV) in each group were measured by uhrasonography in the 28^st day. The relationships among MMP-2, MMP-9 and LVEDV were evaluated. Results: the indexes of the expression of MMP-2 and MMP-9in AMI group were higher than those in control group (P〈0.01) , the indexes of the expression of MMP-2 in 21^st day in AMI group were higher than those in the others day (P〈0.01) , but the indexes of MMP-9 in 28^st day were higher than those in the others day (P〈0.01) ; The LVEDV in the 28^st day of AMI were larger than those in the control group. There was relationship among MMP-9 and LVEDV. Conclusion: MMP-2 and MMP-9 maybe participate the impairing of the AMI, MMP-9 maybe participate the remodeling of left ventricular during acute myocardial infarction.
