交锁髓内钉治疗桡骨骨折的解剖与临床 被引量:3

摘要 目的:观察桡神经浅、深支的形态学特征,以便更安全、方便地开展桡骨交锁髓内钉固定术。方法:观察40套上肢标本中桡神经浅、深支进行形态学特点并进行解剖学测量,并应用于19例桡骨骨折患者的交锁髓内钉手术操作。结果:桡神经浅支在距离Lister结节(36.9±9.7)mm近段桡侧处分出内、外侧两支,在前臂旋前位采用外侧入路置入交锁髓内钉远端锁钉时,锁钉与桡神经深支的最短距离为15.5mm,19例桡骨骨折患者手术操作均安全、顺利完成,无桡神经浅、深支损伤等并发症发生。结论:桡骨交锁髓内钉进钉点选择Lister结节不易损伤桡神经浅支,但选择切口时应注意尽量避开头静脉。桡骨交锁髓内钉远端锁钉在前臂处于旋前位,从外侧入路锁钉时桡神经深支损伤的可能性较小。置入远端锁钉的安全区域为距离肱桡关节面3cm以内。
