缺血预处理对犬肺移植供体肺的保护作用

THE PROTECTIVE EFFECTS OF ISCHEMIC PRECONDITIONING ON DONOR LUNG IN CANINE LUNGT TRANSPLANTATION

以１０例犬左侧单肺移植为基础，观察左侧肺门阻断１０ｍｉｎ再灌注１５ｍｉｎ的缺血预处理结合４ＣＥｕｒｏ－Ｃｏｌｌｉｎ’ｓ液对供体肺进行冷灌及保存，结果表明：采用肺缺血预处理，对供体肺具有更好的保护作用，其作用机制可能与抑制了中性粒细胞的肺内聚集与激活有关。

wenty adult mongrel canines underwent left lung allotransplantation.Donor lungs wereflushed with 4℃ Euro-Collin’s solution(ECS)and stored in the same solution for 3 hours,andthen transplantated to recipient canines.Five donors were treated for ischemic preconditioning(IP)by occuluding left lung hilum for l0 n1in and releasing for l5 min before being flushed(Group IP);five donors were not(Group C).The blood of recipients and donor lung tissueswere collected just after thoracotomy and l hour after transplantated lung reperfusion.The re-sults were:(1)The number of polymorphonuclear(PMN)in lung venous blood were significantlyhigher in Group IP than in Group C,but the number of PMN in lung interstitium under mi-croscopy was less in Group IP than in group C.(2)The TXB2 levels in lung arterial blood weresignificantly lower in Group IP than in Group C.(3)Oxygen tension in lung venous blood wassignificantly better in Group IP than in Group C.(4)Mean pulmonary artery pressure was muchlower in Group IP than in Group C.(5)Histological findings showed less damage in Group IPthan in Group C,It is evident that the protective effects of IP together with ECS flush are superi-or to using ECS alone. Inhibition of PMN accumulation and activation in lung tissue may be oneof the important mechanisms in lung protection.
