纤维支气管镜在机械通气中的应用

Application of fiberbronchoscope in mechanical ventilation.

目的 明确纤维支气管镜（纤支镜）在机械通气患者中的应用价值。方法 在监护下对我院收治的59例机械通气患者行纤支镜检查及治疗。结果 对血氧饱和度下降、呼吸和心率不稳定、气道压力明显不稳定、血痰、肺不张、严重肺部感染等能查找出原因，并予相应的镜下治疗，效果显著；纤支镜引导经鼻气管插管、换管及引导插胃管成功率高、效果好。结论 纤支镜在机械通气患者中的应用可以发挥重要作用。

Objective To investigate the value of applying fiberbronchoscope in patients with mechanical ventilation. Methods Under monitoring, 59 patients of mechanical ventilation in the department of respiratory and ICU were examined and treated with Olympus P40 fiberbronchoscope. Results The reasons of the decrease of oxygen saturation, irregularity of respiration and heart rate, unstability of airway pressure, blood phlegm, atelectasis and serious pulmonary infection could be found out, and the corresponding fiberbronchoscope treatment was performed with good results. There was higher success rate and better effect in the pemasal tracheal intubation, tube changing and nasogastric intubation guided by fiberbronchoscope. Conclusion Fiberbronchoscope could play an important role in the application of mechanical ventilation.
