注射结扎电容场侧切术治疗重度混合痔360例临床观察

Therapeutical Efficacy of the Combined Procedure of Injection, Ligation,Electric Coagulation and Lateral Incision in the Treatment of Severe Mixed Hemorrhoids. Analysis of 360 Cases

为探讨重度混合痔一期手术治疗的可行性及效果，对治疗组360例和对照组350例重度混合痔分别进行了注射、结扎、电容场、侧切术和外剥内扎术治疗，随访5年。结果两组一次性治愈率分别为100％和68．6％；5年复发率分别为3．1％和33．4％；5年肛裂发生率分别为2．5％和12．0％；术后大出血分别为1．1％和4．9％；肛门狭窄分别为0和9．1％；口服止痛药分别为3．3％和47．4％。结果表明，重度混合痔采用注射、结扎、电容场、侧切一期综合手术治疗是可行的，远期效果满意。

The feasibility and efficacy of the combined procedure of injeetion,ligation,eleetrie coagulation and lateral incision（ILCI） in the treatment of severe mixed hemorrhoids were investigated in the study. 360 patients with severe mixed hemorrhoids underwent ILCI（the experiment group）,the efficacy of which was compared with that of 350 patients who had undergone traditional procedure of external dissection and internal ligation（the control group）. Follow-up was done for 5 years. Results showed that the primary cure rate, 5-year recurrence rate, 5-year occurrence rate of anal fissure, postoperative bematorrhea rate, occurrenee rate of anal stricture and postoperative oral intade of analgesics in the experimental group and the control group respectively were 100% vs 68.6%0,3.1% vs 33.4% ,2.5% vs 12.0%,1. 1% vs 4.9%,0 vs 9.1% and 3. 3% vs 47.4%. These suggest that the procedure of ILCI is feasible and the long term efficacy is better,
