影响股骨髁间骨折疗效因素分析

Analysis of relevant factors influencing therapeutic effect of femur intercondylar fracture

目的评价AO解剖钢板固定治疗股骨髁间骨折的疗效。方法采用切开复位AO解剖钢板内固定治疗股骨髁间骨折36例。结果随访16—24个月。术后18—32周骨折全部愈合。采用Merehan等提出的功能评定标准，优20例，良9例，中4例，差3例，优良率80．5％。结论股骨髁间骨折手术前积极止血及消除肌肉水肿，手术解剖复位，手术中严密止血，关节囊重建，术后彻底引流，早期进行功能锻炼，将对股骨髁间骨折愈后疗效起到关键的作用。

Objective To evaluate the results of AO internal plate fixation in treating femur intercondylar fracture. Methods 36 six patients diagnosed as femur intercondylar fracture were treated with open reduction and AO plate internal fixation. Results The patients were followed up for 16-24 months. The fracture was all reduced 18-32 weeks after operation. The outcome was evaluated and analyzed with Merchan rating system. The excellent to good rate of treatment was 80.5% ,including excellence in 20 cases ,better in 9 cases, moderate in 4 cases and poor in 3 cases. Conclusion Preoperative hemostasis and elimination of muscular swelling, intraoperative anatomical reduction and effective control of bleeding and joint capsule reconstruction, postoperative dual drainage and early function exercises are the key prognostic factors of femur intercondylar fractures.
