摘要 目的研究NICO无创心输出量监测系统在ICU中的应用。方法重症监护病房(ICU)15例危重患者,在呼吸机辅助控制通气条件下,采用美国NICO心输出量监测系统,用部分二氧化碳重复吸入法(RB)连续监测无创心输出量(RBco),并同时采用温度稀释法测定心输出量(TDco),比较两种方法所测心输出量值的相关性。结果统计分析显示两种方法测定的心输出量值无显著性差异(P>0.05),相关回归分析显示相关系数为0.90,回归方程为RBco=0.81TDco+1.25,偏离为0.20,精确度为0.78。结论NICO监测系统所测无创心输出量(RBco)与温度稀释法心输出量(TDco)有良好的相关性,NICO无创心输出量监测系统为ICU提供了一种安全可靠的无创心输出量测定手段。 Objective To study the application of Novametrix Medical Systems Inc(NICO) in ICU .Methods Fifteen critical patients receiving mechanical ventilation were studied . Each patients was instrumented with a pulmonary artery thermodilution(TD) cathether(DualTherm, Braun Medical Inc, Bethlehem , PA ), and a partial CO2 rebreathing (RB) ventilator circuit(Novametrix Medical Systems, Wallingford, CT) . Cardiac output was measured simultaneous by RB and TD methods. Paired ttest and regression statistics were used to compare the degree of agreement between concurrent measurement of TDco and RBco. Results Paired t - test analysis revealed no significant difference between RBco and TDco measurement (P〉0.05). Regression analysis gives an r=0.90 with a slop of 0.81 and an intercept of 1.25L/min. Bland-Airman analysis resulted in a bias of 0.20 L/min with a precision (ISD) of 0.78L/min. Conclusion There was a high correlation between RBeo and TDco . Novametrix Medical Systems Ine(NICO)can provide a safe and reliable estimation of noninvasive cardiac output in ICU.
