摘要 目的:研究血管内皮生长因子(VEGF)和增殖细胞核抗原(PCNA)在同一膀胱移行细胞癌(BTCC)组织中的表达及其与BTCC临床、病理的关系,探讨二者与肿瘤血管形成和肿瘤细胞增殖的关系。方法:采用免疫组织化学LDP法对77例BTCC组织中VEGF和PCNA的表达情况进行检测。结果:BTCC组织中VEGF与PCNA阳性表达率分别为66.23%和51.94%,VEGF表达和PCNA表达均与BTCC的组织学分级显著相关(P<0.05);浸润性肿瘤明显高于表浅性肿瘤(P<0.05);术后复发组明显高于未复发组(P<0.01);在VEGF阳性表达的病例中,PCNA表达随VEGF表达强度的升高而升高(P<0.05),且肿瘤复发率也升高,二者呈明显正相关性(P<0.05)。VEGF和PCNA阳性或同时表达阳性的患者复发迅速,PCNA阴性而VEGF阳性表达者较阴性表达者预后差。结论:VEGF和PCNA与BTCC组织学分级、恶性程度和预后密切相关。BTCC是典型的血管依赖性和增殖性肿瘤。VEGF和PCNA均可能通过不同的途径参与了肿瘤血管形成和肿瘤细胞增殖的调控。 Objective: To study the expression of VEGF and PCNA in bladder transitional cell carcinoma (BTCC) tissue and the relationship between these expression and clinicopathological parameters, in order to understand the role of VEGF and PCNA in the angiogenesis of tumor and tumor cell proliferation. Methods:The expression of VEGF nad PCNA in 77 cases of the BTCC were detected by immunohistochemical LDP methods. Resuits: The positive expression rates of VEGF and PCNA in 77 cases of BTCC were 66.23 % .51.94 %, respectively. Both the expression of VEGF and PCNA were related to the tumor pathologic grade and the invasiveness of tumor ( P 〈0.05). The PCNA-positive cells(expression) in VEGF-positive tumor were significantly higher than those in VEGF-negative tumor ( P 〈0. 01). In addition, PCNA-positive cells(expression) significantly increased with increasing VEGF expression( P 〈0. 05). The positive correlation was noted between VEGF expression and PCNA expression( P 〈0.05). Tumor with high VEGF that were positive for PCNA recurred earlier and had a greater risk of stage progression than did those low VEGF and PCNA negative tumors. The high levels of VEGF also indicated the poorest prognosis in PCNA negative than did those the low VEGF tumors. Conclusions: This study indicates that VEGF and PCNA is closely related the histopathological grading,the tumor differentiation and prognosis of BTCC. The pathological changes in the tumor is typically vessel dependency and cell proliferation. VEGF and PCNA may be involved in the regulation to tumor angiogenesis and tumor cell proliferation through the different ways.
