原发性中枢神经系统非霍奇金淋巴瘤临床分析

Clinical analysis of primary central nervous system non-Hodgkin's lymphomas

目的探讨原发性中枢神经系统非霍奇金淋巴瘤(PCNSNHL)的早期诊断、治疗方法及预后。方法回顾性分析86例术后经病理证实为PCNSNHL的临床特征、影像学特点、治疗方法及预后。结果术前早期诊断12例。随访63例,死亡48例,平均生存期13.74个月。术后行放、化疗组,平均生存期14.68个月(最长达5年);未行放、化疗组,平均生存期仅2个月。结论PCNSNHL术前早期诊断有可探讨之规律,尤其是影像学特征;采用包括手术和放、化疗在内的综合治疗是提高疗效的关键。

Objective To explore the early diagnosis, treatment methods and prognosis of patients with primary central nervous system non-Hodgkin＇s lymphoma （PCNSNHL）. Methods The clinical data of 86 cases of PCNSNHL confirmed by postoperative pathological examination were analyzed retrospectively, including clinical characteristics, radiological features, treatment methods and prognosis. Results Twelve cases were diagnosed preoperatively. Sixty-three cases were followed-up, thereinto 48 cases were dead, the overall mean survival was 13.74 months. The mean survival of patients with radiotherapy and chemotherapy was 14.68 months （the longest one was 5 years）, whereas the mean survival of patients without radiotherapy and chemotherapy was only 2 months. Conclusion The early diagnosis of PCNSNHL before operation is feasible, according to patients＇ characteristics, especially the radiological features. Comprehensive treatment including surgery, radiotherapy and chemotherapy is the key to improve the outcomes.
