预防骨科病人发生Ⅱ期压疮的综合管理

The comprehensive management of prevention for stage Ⅱ pressure ulcer in orthopaedic patients

目的通过综合管理,实现骨科病人Ⅱ期压疮发生率为零。方法建立Ⅰ期压疮的分级鉴定标准;评估病人压疮发生的风险;根据评估结果,有针对性地建立压疮预防交班记录;运用规范预防程序落实护理措施;落实压疮发生的奖惩制度。结果提高了护士的责任心及防范压疮意识,促进了病人及家属的积极配合,使骨科病人的Ⅱ期压疮发生率为零。结论只要认真执行预防管理措施,骨科病人Ⅱ期压疮可以实现零发生率。

Objective To achieve the objective of zero rate of stage Ⅱ pressure ulcer in orthopaedic patient through the comprehensive management. Method The following measures were carried out,such as establishing the identification standards of stage I pressure ulcer,assessing the risks of pressure ulcer,establishing nursing records of prevention for pressure ulcer,implementing nursing measures according to the standard prevention procedure,practicing the rewards and punishment system. Result This practice improved nurses＇ sense of responsibility and prevention awareness of pressure ulcer, promoted the positive cooperation of patients and their relatives. As a result,the rate of stage Ⅱpressure ulcer acieved to be zero. Conclusion The rate of stage Ⅱ pressure ulcer can be achieved to be zero through strict implementation of precaution management measures.
