环磷酰胺冲击治疗狼疮肾炎临床观察

Clinical Observation of Impulsion Treating Lupus Nephritis with Cytoxan

目的：探讨环磷酰胺（CTX）冲击治疗狼疮肾炎（LN）的疗效。方法：将病人随机分为三组，A组：CTX每次O．2g，隔日1次静脉推注，总量6—8g后停用；B组：CTX8—12mg／kg静脉滴注，每2周1次，连用2d，总量少于150mr,／kg；C组：CTX0．8—1．0g／次，静脉滴入。每4周1次，持续6个月-1年或病情好转后改为2个月1次，2～3次后病情仍稳定改为3个月1次，以后为半年-1年1次。维持用药2—5年，全部病人同时使用强的松常规治疗。结果：CTX冲击治疗LN缓解率高，每4周冲击给药与隔日给药组的不良反应差异无显著性，复发情况冲击治疗组明显低于隔日用药组。结论：CTX每4周1次冲击治疗LN并维持治疗2年以上有利于及时控制狼疮活动，保护肾功能，预防LN的复发；治疗LN在发病3～6个月内应用CTX是最佳时期。

Objective： To study the effect of impulsion treating lupus nephritis （LN） with eytoxan （ CTX ）. Method：All the patients were divided into three groups randomly . 6 -8g cytoxan were intravenous injection with 0.2g one time for another day in group A. Less than 150mg/kg cytoxan were intravenously gut- tae with 8 - 12mg/kg once for two weeks in group B. After 6 months to 1 year with 0.8 - 1.0g/time once for four weeks in group C and then change the therapy accordingly. Result：There＇ s high cure rate of impulsion treating lupus nephritis with cytoxan. And there＇ s no difference of side reaction between four weeks and once for another day . The recurrence rate in impulsion treating group are lower than that of comparing group.
