西酞普兰合并小剂量奎硫平治疗抑郁症的对照研究

A clinical observation of the effect of Citaprolan with small-dose Quetiapine in depression

目的观察西酞普兰合并小剂量奎硫平治疗抑郁症的临床疗效。方法85例抑郁症患者随机分成西酞普兰组(单用组)和西酞普兰合用奎硫平组(合用组)。共观察8周,于治疗前,治疗后1、2、4、6、8周末采用汉密顿抑郁量表(HAMD),副反应量表(TESS)评定疗效及副反应,并随访1年,观察其1年内的复发率。结果治疗第8周末,西酞普兰合用小剂量奎硫平组疗效显著,合用组与单用组的显效率分别为87.50%和67.50%,差异有显著性(x2=4.588,p<0.05):合用组在1周末起效,单用组在2周末起效;治疗后1、2、4周HAMD评分差异有显著性(x2=5.36,p<0.05);两组的HAMD因子分在焦虑/躯体化和睡眠障碍方面差异有显著性;TESS评分及1年内复发率无明显差异(x2=0.000,p>0.05)。结论西酞普兰合用小剂量奎硫平治疗抑郁症起效快,克服了抗抑郁药本身起效慢的特点,可提高疗效,并能迅速改善睡眠障碍和焦虑/躯体化症状,对复发率无明显影响。

Objective To study the clinical effect of Citaprolan with small-dose Quetiapine in the treatment of depression. Methods 70 hospitalized patients or outpatients who met CCMD-3 criteria of depression were randomly divided into research （Citaprolan and Quetiapine） group and control （Citaprolan） group for the treatment of 8 weeks. The efficacy and side effects were evaluated with HAMD and TESS. The rates of relapse in one year were observed by following up the patients. Results The treatment of Citaprolan with Quetiapine was effective and the tolerance was better after 8 -week treatment. There were significant differences in recovery rate, HAMD of first week, second week, forth week and HAMD gene score of symptom of body disorder, sleep disorder and orexis after treatment between two groups. But there was no difference in TESS and the rate of relapse in one year between two groups. Conclusions Citaprolan with small - dose Quetiapine could improve the efficacy compared with Citaprolan in treating depression, and could improve sleep disorder, orexis and the feeling of body discomfort, Also Quetiapine had no distinct effect on relapse.
