两种心肌肌钙蛋白I定量检测方法对比分析

摘要 目的分析两种全自动检测系统(BayerACS180和DadeBeringXpand)对心肌肌钙蛋白I(cTnI)的检测性能及其对临床的意义。方法通过对不同浓度的临床样本的检测来评估两个系统之间的相关性;通过重复检测不同浓度的样品来评估两个系统的稳定性;另外,通过对高浓度标本进行系列稀释、检测来分析两种系统的灵敏度差异。结果两种系统之间总相关系数为γ=0.996,对低、中和高三种浓度的cTnI检测相关系数分别为0.672、0.961和0.916;ACS180系统对极低浓度的cTnI样品检测CV为4.91%,Xpand则为96.55%,但对另外四种浓度的cTnI样品检测两者的CV值相差不大;对系列稀释的标本,ACS180系统显示出更优的检测灵敏度。结论两种心肌肌钙蛋白I检测系统操作简便、结果可靠,都适于临床应用,而在稳定性、灵敏度方面,ACS180系统有更好的检测性能。 Objective To evaluate the performance of cardiac Troponin Ⅰ (cTnl) assay between two automated systems (Bayer ACS180 and Dade Bering Xpand) and the benefit to the clinical application. Methods Relationship analysis between the two systems by detecting clinical samples with different concentration, stability analysis by detecting reaptedly the samples with different concentration; Sensitivity analysis by diluting serially the high concentration sample and detecting. Results The total correlation coefficient of the two systems is 0,996 (r=0.996), the correlation coefficient of that is respectively for 0.672,0.961 and 0.916in detecting low , medium and high concentration sample;The CV of ACS180 is 4.91%in detecting very low concentration sample, Xpand for 96.55%,for the other four different concentration samples,the CVs of the two systems are almost indifferent ;For the serial diluted sample, ACS180 shows better sensitivity than Xpand. Conclusion s The results of two systems are acceptable to clinical application; and, ACS180 shows better performance in the analysis of stability and sensitivity than Xpand.
