急性呼吸窘迫综合征的无创正压通气治疗

Non-invasive positive pressure ventilation in treating acute respiratory distress syndrome

目的评价无创正压通气(NIPPV)治疗不同病因导致的急性呼吸窘迫综合征(ARDS)的价值。方法20例ARDS患者,分为原发性肺损伤组(9例)和继发性肺损伤组(11例)。比较两组间的APACHEⅡ分值,NIPPV治疗前后动脉血PH值、氧合指数(PaO2/FiO2)、呼吸频率。结果两组治疗前与治疗2h后的PaO2/FiO2、呼吸频率均有显著差异,两组间的A-PACHEⅡ分值无显著差异。结论NIPPV治疗ARDS安全、有效,可以首选。

Objective To evaluate the therapeutic effect of non-invasive positive pressure ventilation （NIPPV） in treating acute respiratory distress syndrome （ARDS） caused by various factors. Methods The total of 20 patients were divided in two groups ： primonary lung injury group （9 cases） and secondary lung injury group （ 11 cases）. The APACHE Ⅱ score between two groups, artery blood PH, oxygenation index （ PaO2/FiO2 ） , repiratory rate before and after NIPPV in the two groups were compared. Results There were sig- nificantly differences in PaO2/FiO2, repiratory rate before and after 2 h of NIPPV in the two groups, and no significantly difference in APACHE Ⅱ scores was observed between two groups. Conclusion NIPPV is a safe and effective method in the treatment of ARDS, and it may be served as the first choice for clinicians.
