充血性心力衰竭患者T淋巴细胞亚群和白介素-6的表达 被引量:2

Study on Changes of T Lymphocyte Subsets and Expression of Interleukin-6 in Patients with Congestive Heart Failure

摘要 目的探讨充血性心力衰竭(CHF)患者T淋巴细胞亚群和白介素-6(IL-6)表达的变化。方法选择NYHAⅡ-Ⅳ级的CHF患者48例作为观察组,30例健康查体者作为对照组。应用流式细胞仪测定全血T细胞亚群数目,并应用酶联免疫吸附法测定血清、培养上清中IL-6的含量。结果与正常对照组相比,(1)CHF患者CD3^+、CD4^+和CD4^+/CD8^+显著降低(P〈0.01),并均与心衰程度呈负相关(r=-0.976、-0.826、-0.881,P〈0.01),CD;显著增加(P〈0.01),与心衰程度呈正相关(r=0.890,P〈0.01);(2)CHF患者血清和培养上清中IL-6显著增加(P〈0.01),并均与心衰程度呈正相关(r=0.988、0.984,P〈0.01)。结论T细胞亚群数目异常和IL-6表达增加可能是CHF患者免疫功能异常的机制之一,调节细胞免疫功能治疗可能改善CHF患者的预后。 Objective To investigate the changes of T lymphocytes subgroup and expression of interleukin - 6 ( IL - 6) in patients with congestive heart failure (CHF). Methods A total of 48 patients with CHF were recruited according to NYHA standard as trial group, and 30 normal persons were enrolled as the control. Flow cytometry was taken in assay of blood T lymphocyte subsets ; and enzyme - linked immunosorbent assay was used in determining the content of plasma IL - 6. Results Compared with normal control group, the numbers of CD3^+ and CD4^+, and the ratio of CD4^+/CD8^+ in CHF patients decreased significantly ( P 〈0. 01), and were negatively correlated with the degree of heart failure ( r = -0. 976, -0. 826, -0. 881, P 〈0. 01 ), the number of CD8^+ increased significantly and was positively related with the degree of heart failure ( r =0. 890, P 〈 0. 01 ) ; the serum and culture supernatant levels of IL -6 were higher ( P 〈0.01 ), and with a positive correlation with the degree of heart failure ( r = 0. 988, 0. 984, P 〈 0. 01 ). Conclusion Abnormality of T lymphocyte subsets and increase of IL - 6 expression in patients with CHF may be one of the mechanisms for dysfunction of cellular immunity in patients with CHF. Regu- lation of cellular immune function may be helpful to improve the prognosis of patients with CHF.
