摘要
目的评估三维可视化技术在岩骨段颈内动脉解剖特点研究中的应用价值。方法将16例无颅底相关疾病病人的增强CT扫描数据以DICOM格式导入三维影像工作站,在Amira软件的虚拟环境下,分析岩骨段颈内动脉与邻近的骨及血管解剖标志的构筑特点。结果岩骨段颈内动脉起始于颈动脉孔颅外端,先上行为垂直部,然后转向前内构成膝部,自膝部直行至颈动脉管内口为水平部,远端折向前上接续于颈内动脉海绵窦段。组合多种三维可视化模型能够显示岩骨段颈内动脉与邻近的骨迷路、内听道等结构的空间位置关系。结论采用基于CT数据的三维可视化技术,可精确识别岩骨段颈内动脉与一些重要颅底解剖标志的毗邻关系,同时也是术前评估手术入路的有效途径。
Objective To evaluate clinical value of three-dimensional visualizing techniques as a means of obtaining anatomic characteristics of the petrous carotid artery. Methods After enhanced CT scanning involving the cranial base, the DICOM files of 16 patients, who were free of cranial base-related diseases, were transferred to the three-dimensional visualization workstation based on a set of Amira software under UNIX platform. Anatomical characteristics of petrous carotid artery in relation with adjacent bony and vascular landmarks were analyzed in the virtual environment. Results The petrous internal carotid artery starts from the extracranial end of the carotid foramen, runs upward firstly to form the vertical segment, then turn anteromedially to make a genu, from which it travels straightly to internal orifice of the carotid artery to form the horizontal segment, distal end of which turn anterosuperiorly to connecting with intracavemous internal carotid artery. Spatial relationships between petrous internal carotid artery and adjacent structures, including bony labyrinthine, internal meatus, and other landmarks were satisfactorily displayed via a combination of various modeling techniques of three-dimensional visualization, Conclusion Three-dimensional visualization based on CT data is a useful means for individualized preoperative assessment of surgical approach, as well as an effective method for assessing the anatomic relationships between the landmarks about the petrous internal carotid artery.
