大黄蛰虫丸对慢性肝病核素肝脏功能显像定量分析结果的影响

Influence of Dahuang Zhechong Pill on quantification analysis of liver function with scintigraphy in patients with chronic viral hepatitis

目的:观察大黄蛰虫丸对慢性病毒性肝炎、肝硬化患者核素肝细胞功能显像定量分析结果的影响.方法:在常规观察肝功生化指标、血清肝纤维化标志物和B超等指标的同时应用核素肝细胞功能显像定量分析法对36例慢性肝病肝硬化患者服用大黄蛰虫丸的疗效进行治疗前后比较.结果:慢性肝病肝硬化患者肝细胞摄取峰值(PV),摄取指数(UI)、摄取速度指数(UsI)均低于正常对照组(18.94±6.80vs29.67±18.98;10.84±5.99vs18.41±13.87;0.58±0.39vs1.23±0.81,P<0.01,<0.05和<0.01),高峰时间(PT),平均残存时间(MRT),排泄速度指数(DSI)分别较正常对照组延迟或降低(16.83±4.89vs10.95±2.79;5.93±2.96vs2.74±0.97;0.07±0.09vs0.18±0.14,P<0.001,<0.01和0.001).大黄蛰虫丸治疗半年后,随着患者肝功生化指标、血清肝纤维化标志物和B超等指标的改善,反映肝细胞代谢功能的核素肝细胞显像包括PT,MRT和相对摄取指数(RUI)等指标亦明显有所改善(9.05±2.03vs15.45±5.16;2.07±0.69vs6.11±3.19;3.64±1.91vs1.62±0.85,P值均<0.01).结论:大黄蛰虫丸不仅对慢性肝病患者肝功生化、肝纤维化标志物、B超等指标有改善作用,而且在核素肝细胞功能显像定量分析方面能够提供好转的临床证据,说明该药具有改善慢性肝炎、肝硬化患者肝细胞代谢功能的作用.

AIM： To observe the effect of Dahuang Zhechong Pill （DZP） on the quantification analysis of liver function with scintigraphy in the patients with chronic viral hepatitis and liver cirrhosis. METHODS： Thirty-six patients with chronic viral hepatitis and liver cirrhosis were treated with DZP and meanwhile underwent quantification analysis of liver function with scintigraphy as well as routine liver function, fibrosis indexes and Bmode ultrasound. RESULTS： The peak value （PV）, uptake index （UI） and uptake speed index （UsI） in the treatment group were significantly lower than those in the normal control group （18.94±6.80vs29.67±18.98, P〈0.01; 10.84±5.99 vs 18.41 ±13.87, P〈0.05; 0.58 ±0.39 vs 1.23 ±0.81, P〈 0.01 ） , but the peak time （PT） and mean residence time （MRT） were longer than those in the normal control group （ 16.83 ± 4.89 vs 10.95 ± 2.79, P 〈 0.001 ; 5.93 ± 2.96 vs 2.74 ± 0.97, P 〈 0.01 ）. And descend speed index （DsI） was lower than that in the normal control group （0.07 ±0.09 vs O. 18 ±0. 14, P〈 0.001 ）. After treatment with DZP for about 6 months, PT, MRT and relative uptake index （RUI） were remarkably improved （9.05 ±2.03 vs 15.45 ±5.16, 2.07 ±0.69 vs 6.11 ±3.19, 3.64±1.91vsl.62±0.85, P〈0.01）. CONCLUSION： DZP could improve the quantification analysis of liver function with scintigraphy as well as routine liver function, fibrosis indexes and B-mode ultrasound in the patients with chronic viral hepatitis and liver cirrhosis.
