早期微创综合疗法与传统外科治疗脑出血的比较观察

Evaluation of the Micro-wound Operation and the Traditional Surgical Therapy with NaozuzhongⅠ for Cerebral Hemorrhage

目的总结近年来山西医科大学第一医院采用早期（6h内）微创手术及术后服用自行研制的纯中药制剂脑卒中Ⅰ号的综合疗法治疗脑出血的临床应用价值。方法脑出血病人120例，入院后随机分到治疗组和对照1组、对照2组。对照1组在行传统外科手术治疗后，行常规西医内科治疗。对照2组在行早期微创外科脑血肿清除术后，仅行常规西医内科治疗。治疗组在早期微创外科血肿清除术后，常规西医治疗基础上加服纯中药制刑脑卒中Ⅰ号，服用1个疗程20d。治疗20d、40d后进行疗效评价及病人术后3个～12个月根据日常生活（ADL）能力进行随访结果比较。结果治疗组总有效率为93．0％和显效率为83．7％，对照1组的总有效率为44．4％、显效率为38．9％，对照2组的总有效率为70．7％、显效率为63．4％．3组间两两比较均有统计学意义（P〈0．05）。随访结果显示：3组中ADL1-ADL3级所占百分比分别为28．1％、53．8％、76．2％，进行组间比较，有统计学意义（P〈0．05）。结论早期（6h）实施微创手术治疗脑出血效果优于传统外科手术治疗脑出血，加用脑卒中Ⅰ号制剂结合早期微创手术的综合治疗脑出血有明显的临床效果。

Objective To study the effect of the micro-wound operation and the traditional surgical therapy with Naozuzhong I for cerebral hemorrhage. Methods One - hundred and twenty patients with cerebral hemorrhage were divided randomly into treatment group （ n = 43） treated by micro- wound operation with traditional Chinese drug Naozuzhong Ⅰ , control groupl （n = 36） treated by the traditional sur gical intervention and routine medical treatment and control group 2 （n = 41 ） by micro- wound operation and routine medical treatment. Results The total effective rates was 93.0 % and the excellence rates was 83.7% in treatment group, that was 44.4% and 38.9% in Control group 1, that was 70.7 % and 63.4 % in control group 2 （P 〈 0.05）. Conclusion The effect of micro - wound operation for cerebral hemorrhage was better than that by traditional surgical intervention. The micro- wound operation with Naozuzhong Ⅰwas the best way for cerebral hemorrhage.
