复方莪倍霜对斑块状银屑病皮损血流灌注的影响

Observation on the Changes of the Perfusion Image of Plaque Psoriasis with Using E-bei Ointment by Laser Doppler Perfusion Imaging(LDPI)

目的评价血流灌注指标与斑块状银屑病皮损症状的相关性,观察复方莪倍霜治疗前、后斑块状银屑病皮损血流灌注的改变情况。方法采用激光多普勒图像诊断系统(LDPI),于复方莪倍霜治疗前、后对靶皮损评分,对靶皮损及周围正常皮肤的血流灌注扫描,用Pearson相关法分析皮损评分与各血流灌注指标的关系,采用配对t检验分析复方莪倍霜治疗前、后的血流灌注改变情况。结果血流灌注增高值(靶皮损血流值与正常皮肤血流值的差值)与皮损单项症状(红斑、鳞屑、浸润)评分及总分均显著相关(P<0.01);复方莪倍霜治疗后,随着皮损评分的降低,血流灌注增高值亦相应降低,治疗前、后比较差异有显著性意义(P=0.006)。结论血流灌注增高值能够更准确地反映斑块状银屑病皮损的症状变化;复方莪倍霜能够改善斑块状银屑病患者增高的血流灌注及临床症状。

Objective To evaluate the dependability between the perfusion image index and symptoms of psoriasis and to compare the changes of index with the usage of E-bei ointment on target lesion. Methods Fifty target psoriatic lesions and the normal skin around were detected by the Laser Doppler Perfusion Image （LDPI） system while the lesions were scored through clinical symptoms. Nineteen lesions were treated with E-bei ointment in a 4-week, openlable, prospective clinical trial. Indexes of LDPI were also measured between baseline and endpoint. Results Difference value between index of lesion and index of normal skin around was significantly correlated to every single symptom score and the sum score （P 〈0.01 ）. With the treatment of E-bei ointmentthe difference values reduced to 0. 15 ±0.21 at the endpoint,while the basement values were 0.32±0. 25 （ P =0. 006）. Conclusion Difference value of LDPI can reflect the changes of clinical psoriatic lesions. E-bei ointment can improve the perfusion image of plaque psoriasisas well as the clinical symptoms.
