摘要
运用漂浮导管热稀释法监测汉防己素(简称汉甲)对10名健康受试者及12例高血压病患者的急性血液动力学作用。结果:对正常人,汉甲减慢心率(HR),升高平均肺动脉压(MPAP)和肺毛细血管嵌顿压(PCWP),而平均动脉压(MAP)、心输出量(CO)、心脏指数(CI)用药前后差异无显著性。对高血压病患者,汉甲明显降低MAP、MPAP、PCWP,明显升高CO、CI,对HR无影响。提示汉甲具有潜在负性变力性与负性变时性效应,对正常血压无影响;但对高血压病患者,汉甲在安全有效降压的同时,能促进左室功能的改善。
Swan-Ganz Catheterization was performed on 10 healthy males and 12 hypertensives with myocardiac hypertrophy.Heart rate(HR),blood pressure(BP),mean pulmonary artery pressure(MPAP),pulmonary capillary wedge pressure(PCWP) and cardiac output(CO) were measured before and after intravenous administration of tetrandrine,and cardiac index(CI),stroke volume(SV),stroke volume index(SVI),systemic vascular resistance(SVR)and mean artery pressure(MAP) were calculated.Intravenous administration of tetrandrine in the 10 healthy males caused decrement of HR and reduction of PAP and PCWP(P<0.05).There were no significant changes in MAP,SVR,CO,CI and SV.Intravenous administration of tetrandrine in the 12 hypertensives caused decrease of MAP,SVR,PAP and PCMP(P<0.01) and increment of CO,CI,SV and SVI(P<0.01);The results suggest that tetrandrine possesses negative inotropic action and does not affect normal blood pressure,but the drug remarkably reduces blood pressure of hypertensives and improve their left ventricular function.
