甲泼尼龙冲击治疗小儿麻疹并重症肺炎的临床观察

A clinical observation of pulse methylprednisolone therapy on severe measles pneumonia in children

目的 观察甲泼尼龙冲击治疗小儿麻疹并重症肺炎的临床疗效。方法 将42并重症肺炎患儿随机分成观察组22例和对照组20例。观察组除接受一般治疗外．采用甲泼尼治疗，初20～30mg／（kg·d）。每12小时一次，治疗3d，后减为5～10mg／（kg·d）。治疗2～3d，改米松0．2～0．3mg／（kg·d），维持2～4d。结果 观察组患儿在呼吸困难、肺部啰音、血气、胸片恢上均较对照组短，差异有统计学意义，且机械通气病例明显减少（P〈0．05）。结论 甲泼尼龙冲麻疹并重症肺炎患儿有效。

Objective To evaluate the efficacy of pulse methylprednisolone therapy （PMT） on severe measles pneumonia in children. Methods Forty-two children with severe measles pneumonia were randomly divided into two groups： PMT group（22 cases） and control group（20 cases）. Both groups were treated by clinical routine therapy, furthermore, twenty-two patients of PMT group received 20-30 mg/（kg·d） methylprednisolone intravenously every 12 hours for 3 days, and then 5- 10 mg/（ kg·d） intravenous methylprednisolone for 2 - 3 days followed by Dexamethasone 0.2 - 0.3 mg/（ kg·d） for 2- 4 days. Results In contrast to the control, PMT group had significantly shorter recovery time of dyspnea, rales, hypoxia and chest x-ray abnormality. PMT group had a lower incidence of mechanical ventilation than the control group （P 〈 0.05 ）. Conclusion Pulse methylprednisolone therapy is effective on severe measles pneumonia in children.
