肺炎支原体肺炎患儿免疫功能动态变化及临床意义

Developmental changes and clinical significance of immunological function in the cases with mycoplasma pneumoniae pneumonia

目的:探讨肺炎支原体肺炎(MPP)急性期、恢复期患儿外周血中血清免疫球蛋白(IG)、T淋巴细胞亚群、C反应蛋白(CRP)和精制结核菌素(PPD)皮肤试验的动态变化及其临床意义。方法:检测35例MPP急性期及其中24例恢复期血清IgA、IgG、IgM和CRP,外周血CD3、CD4、CD8及做PPD试验,并与正常儿童30例进行比较。结果:MPP组急性期及恢复期较正常儿童的血清IgG及IgA水平降低(急性期P<0.01,恢复期P<0.05),CD3及CD4水平降低(均P<0.01),PPD反应无力(P<0.01),CRP急性期较恢复期及对照组增高(P<0.01)。结论:体液免疫和细胞免疫参与了MPP感染的发病过程,且存在免疫功能降低及紊乱。

Objective： To study the developmental changes and clinical significance of serum immune globulin （IG）, T - lymphocyte subsets, C - reactive protein （CRP） in peripheral blood and purified aplisol （PPD） skin test in the cases with mycoplasma pneumoniae pneumonia （MPP） in acute and recovery phases. Methods： Detected serum IgA, IgG, IgM, CRP, peripheral blood CD3, CD4, CD8 in 35 cases with acute - phased MPP and the 24 recovery - phased of them; compared them with those of 30 unimpaired children. Results： The serum IgG and IgA levels （the acute - phased P 〈 0.01, the recovery - phased P 〈 0.05）, CD3 and CD4 （mean P 〈 0.01） in the acute - phased and the recovery - plumed MPP were lower than these in the unimpaired, with responsive asthenia PPD （P 〈 0.01）; the acute - phased CRP increased more than those in the recovery - phased and the controlled （P 〈0.01）. Conclusion： Humoral immunity and cellular immunity involve in the episode of MPP infection, with the presence of decreasing derangement of immunological function.
