超声造影在检测移植肝肝动脉栓塞中的初步应用

Preliminary application of contrast-enhanced ultrasonography in the diagnosis of the hepatic artery occlusion after liver transplantation

目的探讨超声造影在移植肝肝动脉栓塞诊断中的价值。方法对6例移植肝术后怀疑肝动脉栓塞的病例进行超声造影。仪器选用意大利百胜公司生产的DU8彩色多普勒超声仪,机械指数(MI)设置为0．08～0．11。用SonoVue造影剂1．0ml经肘部(?)静脉快速推注(<5s)。结果全部病例均经DSA或临床综合检查确诊。其中明确肝动脉阻塞的4例,未阻塞的2例超声造影诊断移植肝肝动脉阻塞的准确性为100％。结论超声造影对移植肝术后血管并发症的诊断有很大帮助。

Objective To discuss the value of contrast-enhanced uhrasonography （CEUS）in the diagnosis of hepatic artery occlusion （HAO） after liver transplantation. Methods Six patients suspected of HAO were examined by CEUS. Esaote DU8 with contrast enhanced ultrasound technique was used and low mechanical index of 0.08-0.11 was set. A dose of 1.0 ml SonoVue was injected rapidly by antecubital fossa vein. Results all cases were confirmed by DSA or clinically. Of them, 4 cases were HAO and others were patent hepatic artery. The accuracy rate of diagnosing HAO was 100%. Conclusion CEUS technique may be a useful tool in assessing the vascular complications of transplant liver.
