摘要
目的探讨核素骨显像在儿童髋关节疼痛中的鉴别诊断价值。方法采用99mTc-亚甲基二膦酸盐(MDP)骨三相显像以及针孔淮直显像对64例单侧髋关节疼痛伴活动受限患儿进行股骨头显像。结果12例显像结果正常;42例患儿呈滑膜炎表现;10例呈股骨头坏死表现。此10例中平面平行孔显像检出2例,占20%;针孔准直显像检出10例,占100%。结论放射性核素骨显像对儿童滑膜炎与早期Perthes病鉴别诊断敏感,采用三相技术及应用针孔准直器可提高检出率。
Objective To investigate the value of radionuclide bone imaging in differential diagnosis of the childhood patients with hip joint pain. Methods Three-phase bone scintigraphy (blood-flow, blood-pool and delayed phases)and pinhole imaging of the hips were performed in 64 patients with unilateral hip joint pain and hip disorders. Results In the 64 patients studied, radionuclide imaging showed that 12 cases was normal, 42 cases presented as synovitis, 10 cases was the local defect or sparseness of the radioactivity in the femoral head. The positive rate of the planar imaging and pinhole imaging was 20% and 100% in the 10 children, respectively. Conclusion Radionuclide bone imaging is highly sensitive to the differential diagnoses of children's synovitis and early Perthes' disease. Three-phase bone scanning and pinhole imaging may be helpful in increasing positive rate.
