葛根素与依帕司他联合治疗糖尿病周围神经病变疗效观察

Therapeutic Effect of Puerarin Combined with Epalrestat on Diabetic Peripheral Neuropathy

目的:评价葛根素与依帕司他联合应用对糖尿病周围神经病变的疗效。方法:58例糖尿病周围神经病变患者随机分组,治疗组28例,用葛根素400mg+NS 250mL静脉点滴,每日一次,同时用依帕司他50mg,口服,每日3次;对照组30例,单独使用依帕司他50mg,口服,每日3次。两组疗程均为4周,其他治疗基本相同。结果:治疗组:显效7例(25%),有效16例(57.1%),无效5例(17.9%),总有效率82.1%。对照组:显效3例(10%),有效14例(46.7%),无效13例(43.3%),总有效率56.7%。治疗组总有效率明显高于对照组(χ2=4.39,P<0.05)。两组治疗后正中神经、腓总神经和腓浅神经的感觉神经传导速度(SNCV)和运动神经传导速度(MNCV)与治疗前比较有明显提高,差别有显著性(P<0.01);而两组之间比较,则无显著性差别。结论:葛根素与依帕司他联合应用明显提高糖尿病周围神经病变的疗效。

Objective： To explore the clinical effect of combined treatment of puerarin and epalrestat on diabetic peripheral neuropathy （DPN）. Methods： 58 cases of DPN were randomly divided into two groups： ①therapy group consisted of 28 cases （12M/16F）, in which the patients were treated with puerarin 400mg ＋ NS 250 ml by intravenous drip per day, and simultaneously the epalrestat was given （50mg, po, tid） for 4 weeks. ②30 cases（14M/16F） in control group were treated with epalrestat （50mg, po, rid） for 4 weeks. Results： The total effective rate was 82.1% in the therapy group. Of them, 7 cases （25%） were overt efficacy; 16 cases （57.1%） efficacy, inefficacy 5 cases （17.9%）. In contrast, overt efficacy 3 cases （10%）, efficacy 14 cases （46.7%） and inefficacy 13 cases （43.3%） was in the control group separately, the total effective rate was 56. 7%. There was a significant difference on the total effective rate between the therapy group and the control group （χ^ 2 value=4.39,P〈0.05 ）. The level of MNCV and SNCV of median nerve, common peroneal nerve and superficial peroneal nerve in the two groups increased significantly after 4 weeks treatment compared with the before treatment （P〉0.01）. No statistical difference of MNCV and SNCV between the treatment and control groups （P〉0.05）. Conclusions： The results indicate that the shortlterm effects are good and fewer side effects in treatment with puerarin combination with epalresat for DPN; the long-term effect is still not clear.
