摘要 目的探讨snailmRNA和claudin-1mRNA在胃癌中的表达及其与临床病理特征及预后的关系。方法制备组织芯片和cRNA探针,通过原位杂交检测snail和claudin1的mRNA在胃癌和正常胃粘膜组织中的表达。结果胃癌和正常胃粘膜中snail和claudin-1的mRNA表达率差异明显。snailmRNA表达与胃癌的分化、TNM分期、脉管及淋巴结转移呈正相关;claudin1mRNA表达与TNM分期、浸润深度、脉管及淋巴结转移呈负相关。两者均有助于判断胃癌预后,claudin1mRNA还可作为判断胃癌预后的独立风险因素。结论snailmRNA与claudin-1mRNA的表达影响胃癌发展,是指导临床治疗及估计预后的有意义指标。 Objective To study the expression of snail mRNA and claudin - 1 mRNA in gastric cancer and their correlation with clinicpathologic feature and survival time. Methods Tissue microarray (TMA) chips and dig - cRNA probes were prepared. Then snail mRNA and claudinl mRNA were examined by in situ hybridization (ISH) in 100 gastric cancers and 25 normal gastric mucosa tissues. Results The result of in situ hybridization (ISH) revealed that among 25 none - tumor cases, the positive case of snail mRNA and clauidn - 1 mRNA were 16% and 100% ,while among 100 cases,the positive rates of each were 46% and 42% ,respectively. There was significant difference between the two groups( P 〈 0.01 ). Snail mRNA expression correlated with differentiation (r = 0. 203, P = 0. 042 ) , TNM stage ( r = 0. 176 ,P = 0. 027 ) ,vessel invasion ( r = 0. 208 ,P = 0. 038 ) and lymph node metastasis ( r = 0. 299 ,P = 0. 003 ) of gastric cancer ( P 〈 0.05). There were negative relationship between claudinl expression and lymph node metastasis ( r = - 0. 307, P = 0. 002 ) , TNM stage ( r = - 0. 303, P = 0. 015 ) , vessel invasion ( r = - 0. 312, P = 0. 002 ) and invasion depth ( r = - 0. 259, P = 0. 021 ). There was a negative relationship between snail mRNA and claudin - 1 mRNA expression( r = -0. 257,P = 0. 010). Survival analysis suggested that snail mRNA and claudin - 1 mRNA might be used to judge prognosis of gastric cancer and claudinl mRNA might be used as independent risk parameter( OR = 0. 360) for the prognosis of gastric cancer. Conclusions The mRNA of snail and claudin - 1 are essential markers to predict the invasion and metastasis of gastric carcinoma ; they can be used as indexes to direct the clinical treatment.
