早期帕金森病患者的执行功能与脑涨落图研究 被引量:3

<TITLE>STUDY ON EXECUTIVE FUNCTION AND ENCEPHALOFLUCTUOGRAPH TECHNOLOGY IN EARLY PARKINSONIAN PATIENTS

摘要 目的研究早期帕金森病(PD)患者的执行功能与脑涨落图(ET)特点,探讨ET对早期PD的诊断以及对具体认知功能的评估价值。方法对20例早期PD患者,以及20例正常对照者进行认知评定和脑涨落图检测。认知评定工具:言语流畅性测验(VFT)、连线测验(TMT)。脑涨落图(ET)检测:分别检测受试者在静息及执行状态下的脑涨落图。结果早期PD组VFT的家中用品数(P<0.01)及超市商品数(P<0.05)的得分均低于正常对照组,连线测验A和B的耗时数延长(P<0.01),抗干扰能力(TMTB-TMTA)下降(P<0.05)。静息状态下,早期PD组的S1谱、S4系、S11系较正常对照组减低(P<0.05)。正常组执行功能与S11系呈正相关(r=0.486,P<0.05),与S1谱呈负相关(r=-0.463,P<0.05);早期PD组执行功能与S4系(r=0.446,P<0.05)、S11系(r=0.537,P<0.05)呈正相关,与S2谱(r=-0.621,P<0.01)、S3系(r=0.448,P<0.05)呈负相关。结论非痴呆、非抑郁的早期PD患者已存在执行功能障碍,在静息和执行状态下有特征性的ET改变。 Objective To analyze the features of executive function and encephalofluctuograph technology (ET) in patients with early Parkinson' s disease (PD), to investigate the value of ET in diagnosis of early PD and evaluation of executive function. Methods The neuropsychological tests and ET were performed in 20 early PD patients anti 20 age-gendereducation matched normal subjects. The neuropsychological tests comprised verbal fluency test (VFT) and trail making test (TMT). The subjects received ET in resting and executive states. Results The PD patients did worse than the nonnal group in VFT and TMT. Comparing with the normal group, the S1, S4 and S11-pedigree of PD patients declined in the resting state (P〈0.05). We found that the score of VFT in the normal executive state displayed direct correlation with the S11-pedigree ( r =0.486, P〈0.05 ), and inverse correlation with the S1-pedigree ( r =-0.463, P〈0.05 ). The executive function of early PD group displayed direct correlation with the S4-pedigree( r =0.446, P〈0.05 )and S 11-pedigree( r =0.537, P〈0.05 ), and inverse correlation with the S2-pedigrcc ( r =-0. 621, P〈0.01 ) and S3-pedigree ( r =0.448,P〈0.05 ). Conclusions The early PD patients have executive traction impairment. The ETs of early PD have distinctive features in both resting and executive states.
