羟基磷灰石义眼座Ⅰ期与Ⅱ期钻孔效果的比较

摘要 目的比较羟基磷灰石义眼座植入Ⅰ期钻孔与Ⅱ期钻孔放置钛钉的手术效果。方法对101例羟基磷灰石义眼座Ⅰ期钻孔及158例羟基磷灰石义眼座Ⅱ期钻孔放置钛钉者行回顾性研究。结果Ⅰ期与Ⅱ期钻孔者对其外观及活动度均满意,无义眼座暴露、继发感染或栓钉脱出等并发症。Ⅰ期钻孔101例,随访3~50月,义眼的活动度为44.8°±2.9°。Ⅱ期钻孔158例,随防6~60月,活动度为42.4°±3.8°。结论Ⅰ期和Ⅱ期钻孔手术在效果和并发症上无明显差别。 Objective To compare the clinical effect of the procedure involving drilling and inserting a motility peg at the time of hydroxyapatite orbital implant or 6 months after operation. Methods This retrospective study included 101 patients who had drived the titanium threaded sleeve into the hydroxyapatite and placed the titanium fiatheaded peg in the sleeve before implanting orbital and replacement of the fiat - headed peg with a titanium motility/support peg 3 to 7 months later (group Ⅰ ) and 158 patient underwent the drilling of the hydroxyapatite and motility peg plaeement 6 month after operation (group Ⅱ). Information records included postoperative facial appearance, motility of the artificial eye, conjunctival sac discharge and other complication. Results There were no cases of secondary infection, implant exposure or extrusion of the motility peg in both group. The cosmesis and the motility of their artificial eye was acceptable in all patients. The motility of the prosthesis was 42.3° ± 3.7° in group Ⅰ and 42.4± 3.8°in group Ⅱ . The most frequent complication encountered was discharge of conjunctiva sac, which occurred in 23 patients in group Ⅰ and 35 patients in group Ⅱ . 5 patients had exuberant granulation in cojunctive sac. Follow-up ranged from 3 to 50 months in group Ⅰ and 6 to 60 months in group Ⅱ , there was no difference in the clinical effect and complications in two group. Conclusions The hydroxyapatite orbital implant primary procedure drilling give is an effective, efficient surgical option.
