有机磷农药诱发犬中毒及救治过程中心电图的变化特征

摘要 目的在有机磷农药敌敌畏所致犬循环衰竭模型上,观察抗毒荆救治前后EKG变化的特征。方法健康成年雄性杂种犬18只分为3组:对照组6只,氯磷定+阿托品治疗组6只,PVX治疗组6只。麻醉后行气管插管及人工呼吸,雏持呼吸稳定,观察犬循环衰竭及救治过程中的EKG变化。结果①毒荆诱发循环衰竭时可出现寞性心动过缓,窭性心动过速,窦性停搏,Ⅱ、Ⅲ度房室传导阻滞,房颤,室性早搏。室速。室颠等各种心律失常。在导致休克的缓慢性心律失常中,Ⅱ度Ⅱ型度Ⅲ度房宣传导阻滞占83%,寞缓度寞性停搏占17%。②PVX治疗组可在10min内使缓慢性心律失常完全恢复寞性心律,并维持2h。至抢救结束。氯磷定+阿托品治疗组恢复寞性心律需要20~30min,有1例室颠死亡。结论 PVX组抗毒治疗疗效明显优于氯磷定+阿托品组。 Objective It is to observe EKG changes in circulatory failure dogs during phosphate pesticide Dichlorvos poisoning and remedying with counterpoison. Methods 18 male dogs were randomly divided into control group, pyradoxime methyl chloride + Atropine treatment group and PVX treatment group. There were six dogs in every group. The dogs were operated with lifesaver and artificial respiration after anesthesia to keep respiration stabilization. EKG changes of the dogs during circulatory failure and remedy course were observed. Results OVarious kinds of arrhythmias such as sinus bradyeardia, sinus tachycardia, sinus arrest, degree Ⅲ and degree Ⅲ atrioventricular block, atrial fibrillation, premature ventricualr contraction, ventricular tachycardia and ventricular fibrillation could appear when circulatory failure induced by toxic agent. Among the brady-arrhythmias which induced shock, degree Ⅱ Type Ⅱ and degree Ⅲ atrioventricular were 83 %, sinus bradycardia and sinus arrest were 17 %. (2)PVX treatment group could make brady-arrhythmias recovering to sinus rhythm within 10 min and could keep 2 h to reach salvage ending, while pyradoxime methyl chloride + Atropine group needed 20- 30 min to recover sinus rhythm and one cases of ventricular fibrillation died. Conclusion The curative effect of anti-poison therapy in PVX group is better than that in pyradoxime methyl chloride + Atropine group.
