陈旧性股骨颈骨折股骨头活性观察

OBSERVATION OF FEMORAL HEAD ACTIVITY IN OLD FRACTURE OF FEMORAL NECK/

陈旧性股骨颈骨折十分常见，但对股骨头的血供状态研究较少。为此，对１５９例多种原因导致的陈旧性股骨颈骨折患者，采用Ｘ线照片、组织学、电镜、四环素荧光标记和ＥＣＴ等项目观察股骨头活性。结果显示：股骨头缺血坏死在无应力状态下有其自然的修复规律，股骨头塌陷与在活跃的修复过程中受应力作用有关，治疗陈旧性股骨颈骨折应当重视患肢在不负重的情况下渡过修复重建过程。对股骨头外观尚好，而头内结构紊乱者，采用有血供的骨瓣移植。

To understand the femoral head activity in old fracture of femoral neck, 159 cases from 1982 to 1994 were observed through X ray film, pathological sections, transparent electronic microscope, tetracycline label technique and ECT examination. The results showed that under a status without stress, the avascular femoral head tended to be repaired in its natural way. The collapse of femoral head was caused by stress to some extent during the active repair process. Great attention should be paid in treating fracture of femoral neck that load should never be put on the femoral head during the process of fracture healing.
