摘要
目的用显微外科技术切除颅底脑膜瘤,以提高全切率,降低死亡率和致残率。方法本组31例经显微外科治疗颅底脑膜瘤,采用经翼点-颧弓-颞下联合入路,并分析手术方法、结果和并发症。结果肿瘤S im pson’sⅠ、Ⅱ级全切除25例,次全切除4例,大部切除2例。术后恢复良好27例,遗留颅神经麻痹4例,无手术死亡。结论颅底脑膜瘤采用颅底外科技术,明显缩短了肿瘤显露距离,最大限度地减少了脑牵拉,同时配合显微外科技术和电磁刀,可取得良好效果。
Objective To improve the total removal rate of cranial base meningiomas,and to reduce the mortality and morbidity. Methods 31 patients with middle cranial fossa base meningioma were treated by mierosurgery. Tumors were resected by variant transpterion-zygomatic areh-subtemple approach. Results and complications were analyzed retrospectively. Results Total removal of Simpson's 1 to 2 grade was performed on 25 patients,subtotal removal on 4 and partial removal on 2. Good postoperative recovery was noted in 27 patients,skull nerve paralysis in 4 and no death. Conclusion Using skull base microsurgery and electromagnetic field seyetem to resect cranial base meningiomas could shorten the distance of surgical approach and lessen the damage to brain.
