摘要
给１８例恶性室性心律失常患者经静脉非开胸植入单根电极埋藏式心律转复除颤器（ＩＣＤ）。此系统除颤器外壳本身作为除颤电极的阳极，表面积为１０８ｃｍ２，阴极为经静脉植入右室的弹簧除颤电极，除颤器埋于左胸前的皮下囊袋内。结果显示１８例患者除颤阈值均≤２４Ｊ，无严重手术并发症。平均住院时间２．６（２～５）ｄ。平均随访６．４（３～１２３）月，５例患者接受了除颤器的治疗。非开胸植入单根电极ＩＣＤ是一种操作相对简单、除颤效果满意的新一代ＩＣＤ。
Transvenousimplantablecardioverter-defibrilatorsprovidesignificantadvantagesinthetreatmentofpatientswithlife-threateningventriculararrhythmias.However,presenttechnologyrequiresmultipleincisions,andlongoperativetimeforsuccessfulimplementation.Wepresentaneweasy-to-insertunipolartransvenousdefibrila-tionsystem,usedin18patients,males14andfemales4,meanageof62years,withmalignantventriculartach-yarrhythmias.Thissystemincorporatesasinglerightventriculardefibrilationelectrodeanda108cm2surfaceareapulsegeneratortitaniumaloyshelasanactiveelectrodeplacedinleftsubpectoralarea.Al18patientsusingthenewsystemobtainedsatisfactorydefibrilationthreshold(≤24J).Duringthemeanfolow-upof6.4months,5pa-tientsreceivedtreatmentfromtheirdevices.Themeanhospitalstayofthepatientswasonly2.6days.Thisnewu-nipolartransvenousdefibrilationsystemissimpletoinsert,andprovidesasatisfactorydefibrilationthreshold,reli-ablesystem.Itshouldsubstantialyreducethemorbidity,time,andcostofdefibrilatorimplantation.
