颈前路减压植骨带锁钛板内固定治疗颈椎损伤

摘要 目的观察颈前路减压植骨带锁钛板内固定治疗下颈椎损伤的疗效。方法2001-01~2005-04对65例下颈椎骨折脱位并脊髓损伤患者行颈前路减压植骨融合钛板内固定手术。Frankel分级:A级8例,B级14例,C级33例,D级10例,所有患者均有椎管侵占现象。结果术后随访6~12个月,平均10.5个月。59例植骨块于3个月内全部骨性融合,病死3例。生存者脊髓功能均有不同程度改善,Frankel分级平均提高1.15级。结论颈前路减压植骨融合钛板内固定是治疗下颈椎骨折脱位并脊髓损伤的有效方法。 Objective To investigate outcome of anterior cervical decompression combined with bone grafting and Titanium plate fixation for lower cervical injuries.Methods From January 2001 to April 2005,65 patients with lower cervical fracture or dislocation complicated by spinal cord injury were treated with anterior decompression, fusion, and internal fixation with locking Titanium plate. According to Frankel grade, 8 were A, 14 were B, 33 were C, 10 were D. All the cases had invasive phenomenon of vertebral canal. Results All the patients were followed-up for 6-12months (mean, 10.5 months). The bone grafts of 59 cases fused in three months, 3 were dead. Spinal function of the patients recovered at different extents, meanly raised 1.15 Frankel grades. Conclusion Anterior cervical decompression combined with bone frusion and locking Titanium plate internal fixation is an effective way in the treat-ment of lower cervical injuries.
