脐血血气分析、胎心监护、Apgar评分联合监测新生儿窒息 被引量:11

摘要 目的探讨脐血血气分析、胎心监护及Apgar评分的相关性,为预测和诊断新生儿窒息及减低新生儿窒息发生提供重要依据。方法对我院205例新生儿出生后立即采集脐动脉血进行血气分析,结合产前胎心监护及Apgar评分进行统计分析。结果1.Apgar评分≤7分的新生儿与评分≥8分新生儿在血气分析结果中pH值、PaCO2、PaO2、BE值及HCO3-均有显著性差异,(<0.05~0.01)。2.随着pH值下降,Apgar低评分增多,产前胎心电子监护异常发生率也随之增加,新生儿窒息发生率增高。结论联合监测脐血血气分析、胎心监护及Apgar评分可提高新生儿窒息诊断的正确性,降低围产儿患病率和死亡率。 Objective To investigate the correlation of blood gas analysis of the umbilical artery, fetal heart electronic monitoring and apgar rating and prognosticate the occuring of asphyxia of newborn. Method 205 cases were randomly choosed who were term birth and they were given the fetal heart electronic monitoring. The blood gas analysis of umbilical artery was inmeasured delivering. A comparison was carried out among those three results. Results 1. The results of blood gas analysis had notability difference between the group of Apgar rating≤7 and Apgar rating≥8(〈0.05~0.01).2. Along with the descend of pH, the augment of low Apgar rating and the abnormity increased of fetal heart electronic monitoring, the occuring rate with asphyxia of newborn was heighten. Conclusion Combined surpervision blood gas analysis of the umbilical artery, fetal heart electronic monitoring and apgar rating could increase the validity of asphyxia of newborn, reduce the sicken rate and mortality rate.
