肝静脉裂隙在肝脏外科中的应用

Role of hepatic venous fissure in liver surgery

目的探讨肝静脉裂隙在肝脏外科的应用。方法30例背驮式肝移植,10例选择性肝静脉阻断切肝术,3例通过肝后隧道建立绕肝阻断带,行右肝叶切除术。结果三种术式都能通过肝静脉裂隙解剖出肝右静脉根部和左、中肝静脉共干,切断肝静脉或阻断肝静脉根部或建立肝后隧道,完成手术。结论肝静脉裂隙是肝静脉根部的重要解剖部位,要解剖出肝静脉根部、解剖肝静脉裂隙是必须的,也是可行、安全、可靠的。

Objective To investigate the use of hepatic vein fissure in hepatic surgery. Methods The right hepatic lobectomy was performed in 30 patients receiving piggyback liver transplantation, 10 selective hepatic vein occlusion for hepatectomy and 3 establishment of hepatic occluding segment behind the liver. Results The 3 approaches could be used to anatomize the root of the right hepatic vein and common branch of the left and median hepatic vein, amputate the hepatic vein or occlude it or even establish the tunnel behind the liver to accomplish the surgery of right hepatic lobectomy. Condusions Hepatic vein fissure is an important anatomical structure at the root of hepatic vein and it is necessary, feasible, safe and reliable for anatomization of the root of hepatic vein.
