胃肠癌腹膜亚临床转移检测的临床探讨

Detection of Sub-clinical Metastasis of Gastric and Colorectal Cancers to Peritoneal Cavity

目的探讨胃肠癌腹膜亚临床转移的检测。方法对施行手术治疗的32例胃肠癌患者进行术前血清癌胚抗原(CEA)、术中腹腔冲洗液CEA含量的测定,以及腹腔脱落细胞学(ECC)检测。结果胃肠癌浆膜受侵者(S(+)),冲洗液CEA含量增高;ECC(+)时,血清、冲洗液CEA含量明显增高(P<0.05)。结论ECC(+)为亚临床转移者,术前血清CEA、腹腔冲洗液CEA含量高者,为亚临床转移的高危人群,三者联合检测可为胃肠癌腹腔灌注化疗提供理论依据。

Objective： To investigate sub-clinical metastasis in peritoneal cavity from gastric and colorectal cancers. Methods： Isolated tumor cells, Carcinoembryonic antigen （CEA） levels in preoperative serum and peritoneal washings were detected in 32 gastric and colorectal cancers who underwent operations. Results： Peritoneal washings CEA levels were significantly higher in S （＋）. preoperative serum and peritoneal washings CEA levels were higher when ECC（＋） （P〈0.05）. Conclusion： If there is ECC（＋） in the patient with sub-clinical metastasis there were higher CEA levels in preoperative serum and peritoneal washings. Theory for infusion chemotherapy in gastric and colorectal cancers was confirmed by above three measures.
