创面延迟愈合与足甲襞微循环障碍的相关性研究 被引量:5

摘要 目的通过观测创面延迟愈合的热烧伤、电烧伤和化学烧伤患者之足甲襞微循环变化,探讨外周皮肤微循环障碍对烧伤创面愈合的影响。方法选择50例烧伤创面延迟愈合病人(延迟组)和50例烧伤创面正常愈合病人(对照组)。延迟组烧伤面积为(15.3±8.8)%,年龄为(58.32±9.78)岁;对照组烧伤面积为(16.2±9.3)%,年龄为(59.83±9.46)岁。用WX-9B型微循环显微镜及微循环图像处理系统观测两组病人足甲襞微循环十六项指标,对两组结果进行对比分析。结果延迟组与对照组比较,延迟组足甲襞微循环清晰度低,管襻数少,输入、输出枝长度缩短,输入枝、输出枝和襻顶直径小,管襻畸形率高,血流速度减慢,红细胞聚集性增高,襻周渗出明显,乳头平坦者多见。结论创面延迟愈合与外周微循环障碍有显著相关性。 Objective To explore the effect of peripheral skin microcireu]atory disturbance on burn wound union by observing the foot nailfold microcirculatory changes of the patients with delayed wound union after thermal, electrical, and chemical burns. Methods 50 patients with delayed wound union (delayed group) and 50 with normal wound union were enrolled in the study. Patients in delayed group suffered a burned area of 15.3 ± 8.8%, aged 58.32 ± 9.78, while patients in control group suffered a burned area of 16. 2 ± 9.3%, aged 59.86 ±9.46. 16 indexes of foot nailfold microcireulation were observed with wx-9B microciroulation instrument and image-processing system. The results of the two groups were compared. Results Compared with control group, distinctness of foot nailfold microciroulatiun, numbers of vessel loops, lengths of input and output branches, the calibers of branches and vessel loops decreased in delayed group. There were also higher deformity rate, faster blood flow velocity and heavier erythrocyte aggregation. Obvious perivnscular exudation and plain nipples can be easily seen. Conclusion The relationship between delayed wound union and foot nailfoid rnicrociroulatory disturbance is significant.
