敏感性关系妄想综合征精神病理学特点的研究

Psychopathological features in patients with sensitive delusion of reference

目的:观察敏感性关系妄想综合征的精神病理学特点,探讨其诊断归属。方法:应用明尼苏达多相人格问卷(MMPI)、艾森克个性问卷(EPQ)、精神症状量表(MPQ)、DSM-Ⅳ-TR定式临床检查量表(SCID)对30例敏感性关系妄想综合征病人进行测量,并与30例年龄等相匹配的精神分裂症病人、30例社交焦虑病人进行对照研究。结果:敏感性关系妄想综合征病人在MMPI中病态人格(Pd)、偏执(Pa)量表分值高于社交焦虑症病人,抑郁(D)、Pd、精神衰弱(Pt)量表分值高于精神分裂症病人(P<0.05);在EPQ中精神质(P)量表分值低于精神分裂症病人,神经质(N)量表分值高于精神分裂症病人(P<0.05);MPQ中精神病特异性症状(S1)、幻觉(H)量表分值低于精神分裂症病人,抑郁特异性症状(D1)量表分值高于精神分裂症病人(P<0.05)。结论:敏感性关系妄想综合征的精神病理学特点不同于精神分裂症和社交焦虑症,诊断归属应列为一种独立的疾病单元。

Objective： To explore the psychopathological features and the diagnosis standard of sensitive delusion of reference. Methods： Minnesota multiphasic personality inventory （MMPI）, Eysenck personality questionnaire （EPQ）,mental psychiatric questionnaire （MPQ）, and structured clinical interview for DSM-Ⅳ-TR axis I disorders-patient edition （SCID- I/P） were used to measure the psychological status of 30 patients with sensitive delusion of reference,and the results were compared with those obtained from 30 patients with schizophrenia and 30 patients with social phobia. Results：In MMPI,pathological deviance （Pd） and paranoia （Pa） scores in patients with sensitive delusion of reference were higher than those in patients with social phobia（P 〈 0.05）,and depression（D）,Pd,and psychasthenia （Pt） scores in patients with sensitive delusion of reference were higher than those in patients with schizophreni （P 〈 0.05）. In EPQ,compared with patients with schizophrenia, psychoticism （P） score was lower and neuroticism （N） score was higher in patients with sensitive delusion of reference（P 〈 0.05）. In MPQ,compared with patients with schizophrenia,psychiatric specific sympto （S1） and hallucination （H） scores were lower and depressive specific symptom （D1） score was higher in patients with sensitive delusion of reference （P 〈 0.05）. Conclusion：The psychopatho logical features of patients with sensitive delusion of reference are different from those of patients with schizophrenia and social phobia. The sensitive delusion of reference is an independent disease entity.
