泪小点塞在干眼症治疗中的应用

The use of silicone punctum plugs in the treatment of dry eye

目的评价泪小点塞在干眼症治疗中的疗效。方法确诊为干眼症的患者50例67眼,均予以泪小点塞治疗,其中接受单眼治疗患者33例,双眼治疗患者17例,按常规记录治疗前、治疗后1个月和3个月的基础泪液分泌量和泪膜破裂时间,检查的结果按两种方法分组进行统计分析,Ⅰ组45例60眼进行治疗前后自身对照,Ⅱ组30例单眼治疗患者左右眼同期对照。结果Ⅰ组45例60眼泪液分泌试验和泪膜破裂时间均获得明显改善(P<0.01),其中有6例脱离了人工泪液治疗;Ⅱ组30例单眼治疗患者治疗眼较未治疗眼在主观症状、泪液分泌量和泪膜破裂时间方面也有明显改善(P<0.05)。其中7眼因不同原因失访,6眼泪小点塞丢失(6/67,占9%),1眼异物感明显取出泪小点塞(1/67,占1.5%),未归入统计分析。结论使用泪小点塞能够减少泪液排出和丢失,改善干眼症状,是干眼症的一种简单、安全、有效的非手术治疗方法,值得推广。

Objective To evaluate the efficacy of the silicone punctum plug in the treatment of dry eye. Methods Sixty-seven eyes of 50 patients with dry eye received a silicone punctum plug as treatment. Of the. 50 patients, there were 17 males and 33 females, and the average age was 37 years. All patients were followed up for three, mooths. A Schirmerr Ⅰ test and brenk-up time （BUT） were used as measurements before treatment and one and three months after treatment. These measurements were used to evaluate the efficacy of using this treatment for dry eye. Results After inserting the punctum plug, symptoms of dry eye improved in most of the patients. The post-treatment data for the. two parameters were compared to the pre-treatment data that had been acquired. The complete data from sixty eyes were analyzed. The differences between post-treatment and pre-treatment data in group Ⅰ （60 eyes） were highly significant （P〈0.001）. In group Ⅱ （30 eyes） one eye of each patient had received treatment. After treatment, the Schirmer Ⅰ test and BUT results Mowed improvement （ P 〈 0.05 ）. Conclusion A silicone punctum plug reduced tear drainage and relieved the symptoms of dry eye. This is a simple, safe and effective method for the treatment of dry eye and is worth adopting for large-scale use.
