摘要
目的:分析3种动脉弹性功能参数[脉搏波传导速度(PWV)、大、小动脉弹性指数(C1和C2)、压力反射波增强指数(AI)]与脉压(PP)间的关系,寻找能敏感反映动脉弹性改变的指标。方法:选择429例高血压患者,同时检测PWV、C1和C2、AI,根据PP水平分成4组,然后分析PP与动脉弹性功能参数间的相关性,比较不同PP水平组动脉弹性功能参数的差异。观察60例PP≥60mmHg高血压患者,治疗后PP下降程度与各动脉弹性功能参数改善间的相关性。结果:PP与PWV正相关,与C1负相关。在不同脉压水平4组间,PWV、C1﹑C2均有显著性差异(P<0.05),AI值仅在脉压≥60mmHg与<60mmHg间差异有显著性(P<0.05)。PP减小与PWV降低呈正相关,与C1升高呈负相关,而与C2﹑AI改变无显著相关。结论:PWV、C1是敏感反映动脉弹性改善与PP减小的大动脉弹性功能指标。
Objective To analyze the relationship between the different arterial elastic indexes [pulse wave velocity (PWV), large and small arterial elastic indexes(C1, C2) and augmentation index(AI)] and pulse pressure(PP), and to find out the sensitive index for the changes of arterial elasticity. Methods Four hundred and twenty-nine patients with hypertension, whose PWV, C1, C2 and AI were simultaneously measured, and all the patients were divided into four groups according to the PP level. The correlation between the arterial elastic indexes and the PP was analysed. In 60 subjects with the PP more than 60 mm Hg, the relationship between the declined degree of PP and the improvement of arterial elastic indexes was also observed. Results The PP was positively correlated with PWV, and negatively correlated with C1. There were significant differences in PWV, C1 and C2 among the patients with different PP levels, but only in AI between the ones with PP≥60 mm Hg and the ones with 〈60 mm Hg. The declined PP was correlated with the improvement of PWV and C1, but no relationship existed between that and C2 or AI. Conclusions PWV and C1 could sensitively reflect the improvement of arterial elasticity and the decline of PP.
