氨氯地平片治疗慢性心功能不全的临床观察

Efficacy and Safety of Amlodipine in Chronic Heart Failure

目的评价氨氯地平治疗慢性心功能不全的疗效与安全性。方法采用随机对照的方法，将120例慢性心功能不全患者随机分为治疗组和对照组，对照组以洋地黄、利屎剂及血管紧张素转换酶抑制剂等药物治疗，治疗组在对照组治疗基础上加用氨氨地平片（5～10）mg／d。比较2组左室射血分数（LVEF）、左室舒张末期内径（LVDd），血压、心率等指标。结果治疗4周后2组心功能均有显著改善，血压、心率、LVDd降低．LVEF升高，治疗前后比较有显著性差异（P〈0．05）；与对照组相比治疗组收缩压下降、LVEF升高幅度更显著（P〈0．01）。2组患者在治疗期间血常规、电解质及肝、肾功能均无显著改变。结论治疗剂量的氨氨地平是治疗慢性心功能不全较为理想的药物。

Objective To evaluate the clinical effectiveness and safety of amlodipine in the treatment of chronic heart failure. Methods 120 patients with chronic heart failure were randomly divided into two groups （control and treatment group）. Patients in both groups were treated with digitalis, diuretic and angiotensin-converting enzyme inhibitor and patients in treatment group received additional amlodipine. Left ventricular ejection fraction （LVEF）, left ventricular end-diastolic diameter （LVDd）, blood pressure and heart rate were kept under observation. Results After 4 weeks treatment, blood pressure,heart rate, LVDd were significantly lecreased and LVEF significantly increased in both groups（P〈0.05）. The decrease in systolic pressure and increase in LVEF were more significant in the treatment group than in control one（P〈0. 01） ,but there was no significant change in blood test,blood electrolyte,hepatic and renal function. Conclusion Amlodipine at therapeutic dose might be effective and safe for the treatment of chronic heart failure.
