目的:探讨临床药师为患者提供出院带药治疗建议的必要性。方法:根据患者出院所带药物,临床药师为其提供正确、全面的药物治疗建议,如药物的用法、用量、最佳服用时间、合适的疗程、可能出现的药物相互作用、不良反应及注意事项等。结果与结论:临床药师为患者提供出院带药治疗建议非常必要,有助于患者在家安全、有效地进行药物治疗。

OBJECTIVE： To discuss the whys for clinical pharmacists to advise on drug therapy for discharged patients METHODS： Clinical pharmacists keep themselves updated of the take away drugs prescribed to discharged patients, and provide correct and elaborated advises on drug therapy, such as usage, dosage, best time for intake, appropriate treatment course, possible drug interaction, adverse reactions and remarks for attention, etc.RESULTS ＆ CONCLUSION： It is necessary for clinical pharmacists to advise on drug therapy for discharged patients in order to ensure safe and effective medication taken by the patients at home.
