摘要
目的:定量分析2型糖尿病患者周围神经病变(DPN)的神经电生理特点。方法 对200例2型糖尿病患者与130例正常成人进行四肢周围神经的运动单位传导速度(MCV),感觉神经传导速度(SCV),复合肌肉动作电位(CAMP),感觉神经动作电位(SNAP)的测定,并进行对比分析。结果 神经速度异常者126例(检出率63.0%),2型糖尿病组的MCV,SCV,CAMP,SNAP较正常对照组比较有显著差异(P均〈0.05),SCV的异常率(56.3%)高于MCV(50.6%),下肢NCV的异常率(56.9%)高于上肢异常率(50.2%),差异均有统计学意义(P〈0.01)。周围神经病变程度与糖尿病病程有关,与空腹血糖水平无明显的相关性。结论 神经传导速度的检测有助于糖尿病周围神经病的早期诊断。
object : To quantificationally analyse the characteristics of electroneurography in the type 2 diabetic patients with peripheral neuropathy(DPN). Methods: The motor conduction velocity(MCV),the sensory conduction velocity(SCV),the compound muscle action potential(CAMP),and the sensory nerve action potential(SNAP) were measured in 200 diabetic patients and in 130 normal subjects. Results: The electroneurography examination showed that 126 cases(63.0%) had pathological changes in NCV.there are significant difference between the diabetic patients and the normal subjects in the MCV ,the SCV,the CAMP and the SNAP(P 〈 0.05). the requency of abnormality rates in SCV (56.3%) were higher than those in MCV (50.6%).the requency of abnormality rates in lower extremities nerve (56.9%) were higher than those of upper extremities nerve (50.2%),and they all have significant difference (P 〈 0. 0 1).The DPN were associated with the course of disease,and have no correlation with the fasting blood glucose (FBG). Conclusion the measurement of the nerve conduction velocity was helpful in the early diagnosis ofperipheral neuropathy in diabetic patients.
