无泵驱动体外膜肺治疗急性呼吸衰竭犬肺组织内皮素-1前体原mRNA的表达

摘要 目的:观察双侧股动脉-股静脉无泵驱动体外膜肺(bAV-ECMO)治疗急性呼吸衰竭犬肺组织内皮素-1前体原和内皮素-1(ET-1)的表达.方法:10只犬在建立急性呼吸衰竭模型情况下给予bAV-ECMO治疗.分别于模型建立完成时、转流开始后1,2,3,4h取肺组织.应用放免及RT-PCR技术,观测bAV-ECMO期间肺组织匀浆ET-1变化、肺组织ppET-1mRNA的转录表达变化.结果:犬ARDS模型肺组织匀浆ET-1含量转流2h达到高值,之后缓慢下降.方差分析表明肺组织匀浆ET-1的均数在转流前后差异有统计学意义.肺组织匀浆ET-1转流3h与4h差异无统计学意义.RT-PCR对ppET-1mRNA的结果与肺组织匀浆ET-1放免结果相一致.结论:对于急性呼吸衰竭bAV-ECMO的早期治疗有积极意义. AIM- To study the dynamic change of the pre-pro- ET-1 (ppET-1) and endothelin-1 ( ET-1 ) in lung tissue of dogs with acute respiratory failure (ARF) treated by bilateral femoral arteriovenous extracorporeal membrane oxygenation ( bAV- ECMO). METHODS: A total of 10 ARF dogs were treated by the bAV-ECMO. Lung tissues were collected at different time (when models were established, 1, 2, 3, 4 h during ECMO). ET-1 was detected dynamically by radio-immunological method and ppET-1 mRNA expression by RT-PCR method during bAV- ECMO. RESULTS: ET-1 content in lung homogenate get to the peak at 2 h and then started to decrease. The analysis of variance showed it was notably different before and after bAV-ECMO, but not different between 3 and 4 h during treatment. The RT-PCR result was in accordance with the radio-immunological result. CONCLUSION: The bAV-ECMO has a positive significance in the early treatment of ARF.
