原发性输尿管癌18例诊治分析

Diagnosis and treatment of primary ureteral carcinoma

目的探讨原发性输尿管癌的诊断与治疗。方法对18例原发性输尿管癌患者的临床资料进行回顾性分析。结果术前15例患者经B超、静脉肾盂造影、膀胱镜及逆行肾盂输尿管造影、输尿管镜等检查诊断为原发性输尿管癌。18例患者均经术后病理检查确诊为原发性输尿管癌。其中15例行根治性患侧肾、输尿管和膀胱袖状切除手术治疗,3例行保守性手术。结论联合应用多种检查方法可以提高原发性输尿管癌的诊断正确率,患侧肾、输尿管和膀胱袖状切除仍是治疗原发性输尿管癌的最重要方法。

Objective ： To investigate the diagnosis and treatment of primary ureteral carcinoma, Methods ： The clinical documents of 18 cases of patients with primary ureteral carcinoma were retrospectively reviewed. Results ： 15 cases of patients were diagnosed as primary ureteral carcinoma by B-ultrasonography, intravenous pyelogram （IVP）, cystoscopy and retrograde urography, CT, MRI, ureteroscopy before operation, all of the patients were proved to be transitional cell carcinoma by pathological examination after operation. Total nephroureterectomy was performed in 15 cases of patients, conservative excision was performed in only 3 cases of patients . Conclusion ：The diagnostic accuracy rate of primary ureteral carcinoma may be improved by combination of various kinds of tests. Total nephroureterectomy is still the most important way in the treatment of primary ureteral carcinoma.
